Wisconsin Rapids Wisconsin DMV Reviews and Tips
The staff was helpful, the wait was short but I went right away in the morning. The location and parking were both easy to find. They have removed chairs and you have to wear a mask and maintain proper spacing.

How can I get a Wisconsin Id for my elderly father?
To get a non-expiring Wisconsin ID for your elderly father, who is 65 years old or older and a U.S. citizen, follow these steps:
1. Obtain a Wisconsin Identification Card (ID) application MV3004. You can access the interactive identification card guide online to help you with the process, pre-fill the application, and check your eligibility for electronic submission.
2. Complete the MV3004 application form by answering all the questions.
3. If eligible, you can submit the application online or schedule an appointment with the DMV for expedited service. Otherwise, you can mail the completed application to WisDOT, P.O. Box 7995, Madison, WI 53707-7995.
4. If you choose to visit a DMV customer service center, bring the following documents with you:
a. Your father's current ID card or other acceptable proof of identity
b. Your father's Social Security number
Please note that if your father has a driver's license, he must surrender it and his driving privileges to obtain the non-expiring ID card. If he has a REAL ID, he must surrender that feature as well, since REAL ID compliant cards follow an eight-year renewal cycle. The non-expiring ID card will have the same appearance and security features as the traditional eight-year cards, but with the words "Non-expiring" in place of the expiration date.
The ID card is free if used for voting purposes. After applying, a receipt with your father's photo will be provided, which can be used as photo identification for voting until the card arrives in the mail.
