    Hello,

    I am sorry to hear about this incident. If your handicap placard was stolen, you are required to report it immediately to your local law enforcement agency as well as your County Motor Vehicle Office. You will need to complete Form number DR 2283 Lost or Stolen License Plates/Permits Affidavit.

    To apply for a replacement handicap placard in Colorado, you will need to visit https://mydmv.colorado.gov/_/ and follow the directions on the application.
