New York DMV Warns of License Suspension for Residents Failing to Submit Vision Test Reports

The New York Department of Motor Vehicles (DMV) has issued a warning to state residents who renewed their licenses online between March 1, 2020, and August 31, 2021, without submitting a vision test report. These individuals are at risk of imminent license suspension and must submit their vision test results immediately to avoid penalties.

The warning targets those who self-certified their vision during the online renewal process but have not yet submitted a completed vision test report to the DMV. The DMV has outlined steps to guide residents in submitting their vision test results.

Step 1: Pass a vision test at an approved provider location. Approved providers can directly enter test results into the DMV's Vision Registry. Alternatively, residents can visit a non-approved provider, who must complete a Vision Test Report (MV-619) for submission to the DMV.

Step 2: Submit the completed vision test report either electronically or by mail. To submit electronically, fill out and submit a short webform using the information from the MV-619. To submit by mail, send the completed MV-619 to:

Residents can also visit their local DMV office to have their vision tested in person.
